BOARD OF SUPERVISORS CONSENT AGENDA:
The following items listed under CONSENT AGENDA will be considered aa' a group and acted upon by
one motion with no separate discussion of said items, unless a Board Member so requests. In that event,
the item will be removed from the CONSENT AGENDA for separate discussion and action.
5. BOARD OF SUPERVISORS SITTING AS THE BOARD OF DIRECTORS - MOHAVE
COUNTY FLOOD CONTROL DISTRICT: Set a Public Hearing for November 6, 2000,
regarding the proposed revised Mohave County Flood Control Ordinance.
6. Award of RFP No. 00-P-11 - Long Distance services to Citizen's Communications, Kingman,
Arizona.
7. Approval of the monthly report for Procurement Activity between $10,000 and $25,000.
8. Accept the recommendation of the Public Land Use Committee to send correspondence to the
Bureau of Land Management of the County's intention to participate in the planning process for
the Grand Canyon-Parashant National Monument.
9. Reappointment of Richard Weiss and Julie Roth as Judge Pro Tempores, and Charles W. Gurtler
as Juvenile Court Judge Pro Tempore for the Superior Court of Mohave County.
10. Approval of promotion of Deputy Don Bischoff from 35-13 to 41-10 retroactive to October 7,
2000, as requested by Chief Deputy Jim McCabe, Mohave County Sheriff's Office.
11. Approve the decrease of Mohave Educational Services Cooperative budget by $463,000.
12. BOARD OF SUPERVISORS SITTING AS BOARD OF DIRECTORS - MOHAVE
COUNTY LIBRARY DISTRICT: Accept donation of $4,000, from Lake Havasu City Friends
of the Library, for purchase of computer furniture, shelving and signage, for the Mohave County
Library in Lake Havasu City.
13. BOARD OF SUPERVISORS SITTING AS BOARD OF DIRECTORS - MOHAVE
COUNTY LIBRARY DISTRICT: Accept donation of $1,000, from Ernest Lockwood, for
purchase of computer furniture, for the Mohave County Library in Lake Havasu City.
14. BOARD OF SUPERVISORS SITTING AS BOARD OF DIRECTORS - MOHAVE
COUNTY LIBRARY DISTRICT: Accept donation of $2,000, from Ford Motor Company, for
seed money toward purchase of a mini-van to begin the Mohave County Library Book-Stop
Service Shuttles project.
15. Approval of tax exemption waivers per ARS 42-1153B, as requested by Beverly Payne, Mohave
County Assessor.
16. Approval of Special Event Liquor License application for the Golden Shores Offroaders, 13136
Golden Shores Parkway, Topock, AZ, on December 9, 2000.
17. Approval of Special Event Liquor License application for the Golden Shores Civic Association,
13136 Golden Shores Parkway, Topock, AZ, on November 10, 2000.
18. Approval of Special Event Liquor License application for the VFW Post 6306, 12852 Oatman
Topock Highway, Topock, AZ, on November 4, 2000.
19. Acknowledge receipt of, and set for Public Hearing, the request from Charter Communications
for a Cable System License, and authorize the Clerk of the Board, as the designated
representative of the Board of Supervisors, to conduct the Public Hearing, on November 13,
2000, at the Beaver Dam/Littlefield Fire Station, Beaver Dam, Arizona.
20. BOARD OF SUPERVISORS SITTING AS THE BOARD OF DIRECTORS - GOLDEN
VALLEY IMPROVEMENT DISTRICT NO. 1: Adoption of Golden Valley Improvement
District Resolutions 2000-01 through 2000-11 - Accepting Petition to incur expense for Phase
IV water line extensions.
21. BOARD OF SUPERVISORS SITTING AS THE BOARD OF DIRECTORS - MOHAVE
COUNTY TELEVISION IMPROVEMENT DISTRICT: Authorize the execution of a Co-
Location Agreement between Mohave County and Mohave Cellular at the Dolan Springs
translator site.
22. BOARD OF SUPERVISORS SITTING AS THE BOARD OF DIRECTORS: MOHAVE
COUNTY TELEVISION IMPROVEMENT DISTRICT: Authorize the execution of a Co-
Location Agreement between Mohave County and Mohave Cellular at the Patterson Slope
translator site.
23. Approve the decisions of the Employee Benefits Trust (EBT) Board to renew contracts for Delta
Dental Plan of Arizona, Inc., Vision Service Plan of Arizona, Inc., Rocky Mountain Life
Insurance Company (for basic life insurance), and Guarantee Life Insurance Company (for
voluntary life insurance) for an additional one-year term beginning January 1,2001, and ending
December 31, 2001, and to authorize the Human Resources Officer to sign any necessary
renewal documentation.
24. Approve the decision of the Employee Benefits Trust (EBT) Board to replace the current fully-
funded BlueCross BlueShield medical insurance plan with a partially self-funded medical
insurance plan and to authorize the premium and contribution rates as adopted by the EBT Board
for the period of January 1, 2001, through June 30, 2001.
25. Award of RFP No. 00-P-12 - Employee Benefits Third Party Administrator (TPA) on behalf of
the Mohave County Employee Benefits Trust (MCEBT).
26. Award of RFP No. 00-P-14 - Medical Benefits Provider Network - on behalf of the Mohave
County Employee Benefits Trust (MCEBT).
27. Award of contracts for Professional Services - on behalf of the Mohave County Employee
Benefits Trust for a Prescription Benefit Manager (PBM) and a Pre-Certification/Utilization
Review Provider (Pre-Cert/UR).

28. Discussion and possible action RE: Evaluation of a request for a REZONE of Parcel 578,
STAGECOACH TRAILS AT SANTA FE RANCH, Unit 7, in Section 25, Township 16 North,
Range 17 West, from A-R/36A (Agricultural-Residential/Thirty-six Acre Minimum Lot Size)
zone to A-R/8A (Agricultural-Residential/Eight Acre Minimum Lot Size) zone, in the Yucca
portion of the Mohave County General Area (west of Alamo Road and north of Red Cloud
Drive), Mohave County, Arizona. (COMMISSION APPROVED BY A 3-2 VOTE -
RESOLUTION NO. 2000-412).
*29. Discussion and possible action RE: Approval of proposed settlement of G.B. Investment
Company, an Arizona corporation, and Bashas' Inc., a corporation, v. Mohave County, TX 99-
00406 tax appeal. This settlement provides for a reduction in the full cash value of commercial
property from $1,669,090 to $1,350,000, resulting in an estimated tax reduction of $4,889.94
(from $44,367.59 to $39,477.65) for the 2000 and 2001 tax years.

VALLEY IMPROVEMENT DISTRICT: Discussion and possible action RE: Request from
the Golden Valley Improvement District #1 Water District residents to address Water Districts
rates and policies.
